Secure Upload Portal
Please use this form portal to safely and securely upload any benefit, new hire, banking, etc. documents you need to submit to the Employee Engagement Office:Â
*Click on the PDF fillable form link you wish to submit to the secure portal for processing, select »å´Ç·É²Ô±ô´Ç²¹»åÌý´Ç°ùÌýsave as (right click your mouse) and save this form to a folder on your computer. Once the form is saved/downloaded on your computer, close the form. Open the form you saved on your computer, complete the form and save it again.
Once you upload your form via the portal, please email the hr@daemen.edu to let us know the form has been submitted.Â
